Job Title: RN or LPN Wound Care Nurse

Location: ASVH Flagstaff

Schedule:
Flexible 8-hour shifts MondayFriday (no weekends): 7 AM3 PM 8 AM4 PM or 8 AM5 PM

Position Summary:

We are seeking one skilled RN or LPN to provide comprehensive wound care management including assessments treatment and education for residents. The nurse will serve as a resource to the care team ensure proper documentation and maintain up-to-date knowledge in wound ostomy and continence care.

Key Responsibilities:

• Perform wound assessments reassessments and develop individualized management plans.

• Develop and implement skin care programs to prevent breakdown.

• Revise care plans based on resident response and evaluate effectiveness.

• Assess and manage pain; educate residents and families on pain management.

• Provide staff in-services on wound prevention wound care ostomy care and incontinence.

• Communicate effectively with nursing staff and physicians.

• Consult with interdisciplinary team members to address resident needs.

• Maintain accurate documentation in line with standards and policies.

• Participate in training and continuing education to stay current with wound care best practices.

Requirements:

• Current Arizona RN or LPN license.

• Certification: CWOCN (Wound Ostomy Continence) or CWCN (Certified Wound Care Nurse).

• Ability to obtain/retain an Arizona Fingerprint Clearance Card.

• Current CPR/BLS certification.

• Preferred: Experience in skilled nursing/long-term care.
