Job Title: Registered Nurse (RN) – Outpatient Dialysis CWR

Location: Neptune, NJ 07753

Job Summary:  The Registered Nurse (RN) is responsible for evaluating patients and assisting physicians with patient care in an outpatient dialysis setting. The RN provides patient-centered care, ensuring quality treatment and supporting evidence-based practices in alignment with professional nursing standards.

Duties & Responsibilities:

• Plan, implement, and document nursing care based on patient needs.

• Identify patient needs and determine priorities for care through assessment, planning, intervention, and evaluation.

• Develop, implement, and coordinate individualized care plans through initial and ongoing assessments.

• Initiate assessments based on patient problems/needs, physician medical diagnoses, and functional health frameworks.

• Perform therapeutic nursing interventions and manage patient care.

• Document care in accordance with standards of practice, patient acuity, and clinical priorities.

• Advocate for evidence-based care standards and support innovations in care delivery.

• Provide patient-centric care in alignment with nursing professional practice models.

• Perform additional duties as assigned.

Required Skills & Experience:

• Minimum of 1 year prior experience in an outpatient dialysis center.

• Minimum of 1 year experience with In-Center Hemodialysis (ICH).

• Knowledge of Fresenius T Machines and portable Reverse Osmosis (RO) systems.

Preferred Skills & Experience:

• Telemetry and chronic hemodialysis experience.

• Ability to manage 3-patient assignments for initiation and termination of dialysis.

• Ability to manage a 9-patient assignment with 2 PCTs for medications and assessments.

Education:

• Required: ASN (Associate of Science in Nursing)

• Preferred: BSN (Bachelor of Science in Nursing)

Licenses & Certifications:

• Current BLS certification (AHA).

• Active NJ State RN license or multi-state compact license.

Schedule & Hours:

• Full-time, 40 hours per week.

• 4 days per week, 10-hour shifts (6:00 AM – 4:30 PM).

• Every other weekend required.

• Holidays required.

• No on-call or float requirement.

Experience Requirements:

• Minimum of 2 years outpatient dialysis experience.

• Experience with Fresenius dialysis machines preferred.

Soft Skills:

• Strong patient care focus in a dialysis setting.
