Job Description

Are you a compassionate and experienced Registered Nurse looking for a unique opportunity to provide exceptional care in a patient-centered environment? We're seeking dedicated RNs to join our pioneering Hospital at Home™ program, bringing healthcare directly to individuals in their own homes. This role allows you to make a significant impact on patients' lives by delivering high-quality, personalized care within their familiar surroundings.

About the Role:

As a key member of our home-based care team, you'll collaborate with a multidisciplinary group of healthcare professionals, including physicians, dietitians, case managers, and pharmacists. You'll play a vital role in coordinating and delivering comprehensive medical care to patients with acute conditions, ensuring their comfort and well-being.

Key Responsibilities:

Seamless Transitions: Partner with hospital providers to identify eligible patients and facilitate smooth transitions to our at-home care program.
Team Collaboration: Participate in daily team huddles to communicate patient capacity and operational updates.
Care Coordination: Manage patient care schedules, diagnostic procedures, and the delivery of necessary medical equipment.
Acute Care Management: Triage and address patients' acute needs and challenges, coordinating efforts with the care team.
Direct Patient Care: Provide hands-on medical-surgical level care to patients in their homes.
Interdisciplinary Collaboration: Work closely with physicians, paramedics, diagnostic services, and remote patient monitoring technologies.
Patient-Centered Approach: Identify and address patient and family needs, ensuring a holistic and supportive care experience.
Quality Assurance: Adhere to quality standards, participate in quality monitoring, and contribute to continuous improvement initiatives.
Case Review: Participate in ongoing case reviews to identify and address any delays or variances in care.
Medical Necessity: Utilize established medical necessity criteria to ensure appropriate patient placement.
Work Schedule and Benefits:

Flexible Scheduling: Choose between (4) 10-hour shifts or (3) 12-hour shifts per week, with scheduling flexibility based on patient needs. Typical shift times are 11AM to 11:30PM or 1PM to 11:30PM.
Weekend and Holiday Rotation: Participate in an every-other-weekend and every-other-holiday rotation.
On-Call Rotation: Participate in an on-call rotation (details to be discussed during the interview).

Qualifications:

Required: Minimum of 2 years of Registered Nurse (RN) experience.
Preferred: Acute care or inpatient nursing experience.
Required: Graduate from Specialty Training Program.
Preferred: Bachelor's Degree in Nursing.
Required: Current RN license in the state of Pennsylvania.
Required: Basic Life Support (BLS) certification.
